What I will do and achieve
Are you passionate about empowering people to live independently and achieve their goals? Working with an allocated group of tenants, you’ll support individuals experiencing mental ill health to promote independence and sustain tenancies. You’ll assess needs and risks, working in a person-centred, recovery-focused way to help people build confidence and thrive in the community.
- Provide key worker support, including one-to-one assistance and liaising with mental health agencies.
- Use communication, motivation, and coaching to help individuals achieve personal goals.
- Build trust and rapport based on respect and honesty, maintaining professional boundaries and following the Rethink Mental Illness Code of Conduct.
- Support day-to-day service operations, including property re-lets, processing referrals, and ensuring health and safety across schemes.
- Encourage engagement in social and workplace activities, helping individuals regain confidence and independence.
Essential skills and experience
- Experience supporting people with mental illness or in a social care setting (paid or voluntary)
- Good listening skills, empathy, understanding, kindness, patience, and a willingness to learn.
- Ability to demonstrate behaviours consistent with the Charity's values.
- Excellent administration, organisation and planning skills.
- Competent in the use of IT systems including Microsoft applications, email and internet.

At Bath Road, we provide more than just accommodation – we offer support with daily living skills, personal care, finances, and rehabilitation, while encouraging engagement with the local community.
What will you do?
- Help individuals rebuild confidence and independence

Care Navigation services aim to provide a seamless package of support to individuals leaving secure settings and returning to the community, reconnecting individuals with health services to reduce health inequalities and in turn reduce reoffending. The Care Navigators are embedded in integrated mental health teams and community settings, providing 1:1 and group assessment and support in a variety of settings and hold highly autonomous roles.
*The applicant must have a full UK Driving Licence and access to a vehicle.
What I will do and achieve
Join our team as a Care Navigator, working with colleagues to support people with health vulnerabilities as they move from prison back into the community. Using a coaching approach, you’ll help maintain wellbeing before and after release, strengthen links with community health services, and reduce the risk of ill health and reoffending. Your work will ensure a smooth transition and help reduce health inequalities for those leaving the criminal justice system.

About the role
Location: HMP Millsike
Working hours: Full-time, 35 hours per week
Rethink Mental Illness work alongside Tees, Esk and Wear Valley NHS Foundation Trust (TEWV) and Humber Teaching NHS Foundation Trust to deliver an integrated stepped care mental health service within HMP Millsike.
The team compromises a range of highly skilled professionals who work collaboratively to provide the highest quality of care for those with mental health needs within the prison and for Rethink Mental Illness employees, this is carried out between 0800 – 1600 Monday to Friday.
Purpose of the Role
To provide person-centred support for individuals with health vulnerabilities transitioning from prison to the community, ensuring continuity of care and effective release planning. The role focuses on reconnecting individuals with health services, improving wellbeing, reducing health inequalities, and supporting a sustainable reduction in reoffending through coaching-based interventions and collaborative working with partner agencies.
What I will do and achieve
Delivering Person-Centred Support
- Provide holistic care navigation to individuals before and after release from prison, ensuring continuity of care and effective, well-planned transitions into the community
- Complete initial screenings, care mapping assessments, and structured release plans using a tailored, person-centred approach
- Deliver flexible 1:1 and group-based support using a coaching framework to promote independence, resilience, and long-term wellbeing
Improving Health and Reducing Reoffending
- Reconnect individuals with appropriate health and community services, reducing health inequalities and supporting sustained recovery
- Prevent deterioration in physical and mental health by enabling access to ongoing care and promoting self-management
- Contribute to reducing reoffending by supporting stability, engagement, and positive lifestyle changes
Collaborative and Multi-Agency Working
- Work closely with prison staff, healthcare providers, and statutory and non-statutory agencies to coordinate safe and effective release planning
- Build and maintain strong professional relationships, representing both the service and service users in multi-disciplinary settings
- Support health promotion initiatives and joint working to improve service reach and impact
Quality, Performance and Accountability
- Maintain accurate, timely, and professional clinical records in line with organisational and national requirements

About the role
As a Peer Support Worker, you’ll be part of a multi-agency Living Well team, working alongside professionals and community partners to provide meaningful, person-centred support.

- Help people connect with their communities and local support services

The team compromises a range of highly skilled professionals who work collaboratively to provide the highest quality of care, for those with mental health needs within the prison and for Rethink Mental Illness employees, this is carried out between 0800 – 1600 Monday to Friday.
As we work across both sites, there may be some travel involved, but this will be done in work time and expenses paid to support service delivery.

What I will do and achieve
Your work will make a real difference by helping people in prison take positive steps toward recovery and a better future. You’ll play a vital role in supporting mental health, reducing risk, and empowering individuals to manage their wellbeing.
In this role, you will
- Assess and manage referrals – Accept referrals via agreed protocols, determine suitability, and ensure clients receive the right level of care, including stepping up treatment when necessary.
- Support recovery and self-management – Work with individuals to identify their goals, assess risk, and empower them to manage their mental health effectively.
- Deliver evidence-based interventions – Provide low-intensity treatments such as Guided Self-Help and psycho-educational support, drawing on CBT principles.
- Collaborate with a multi-disciplinary team – Attend meetings, share insights, and ensure smooth step-up and step-down arrangements for clients.
- Maintain high standards of clinical practice – Record outcome measures, keep accurate data, and uphold confidentiality at all times.
- Contribute to service performance – Meet agreed activity targets, minimise waiting times, and support monthly performance reporting.
- Build strong partnerships – Work with statutory and non-statutory agencies to ensure continuity of care post-release.

Essential Skills and Experience
- Successfully completed academic training aligned with the IAPT Low Intensity National Programme and qualified as a Psychological Wellbeing Practitioner (PWP), e.g., Postgraduate Diploma in Psychological Therapies.
- Accredited PWP with BACP or BPS.
- Post-qualification experience delivering the full range of evidence-based IAPT low-intensity interventions at Step 2.
- Proven experience working within a stepped care service, supporting individuals with mental health problems and achieving recovery outcomes.
- Strong knowledge of assessment procedures, screening, referral processes, record-keeping, and evidence-based brief psychological interventions.
- Understanding and application of evidence-based psychological therapies relevant to this role.

The service will be integrated with CNWL to work as one multi-disciplinary team to deliver services to people who are known to CNWL services.
The service co-ordinator will work alongside two other co-ordinator’s and will have a minimum of 12 volunteers at any one time to deliver the befriending aspects of the service.

As a Befriending and Volunteer Coordinator, you would work in our Operations Directorate - Community Services Division.
You would work within a team which provides high-quality, safe and person-centred support in the community, most importantly delivering great outcomes for people bereaved by suicide, working with the following people and teams

About the role
Location: HMP Millsike
Rethink Mental Illness work alongside Tees, Esk and Wear Valley NHS Foundation Trust (TEWV) to deliver an integrated stepped care mental health service within HMP Millsike.
The team compromises a range of highly skilled professionals who work collaboratively to provide the highest quality of care for those with mental health needs within the prison. Core working hours may vary depending on the service need and provision in different sites (for example, 8am-4pm, 9am-5pm).
What I do and achieve
Join our dedicated mental health team to support the wellbeing of residents in custody. You’ll work collaboratively with stakeholders to deliver holistic care, including release planning, group workshops, and peer support initiatives. Your role will involve:
- Providing mental health resources and self-help materials.
- Facilitating group sessions and service user forums.
- Coaching individuals to set goals and build confidence.

We’re looking for an experienced and passionate Senior Individual Giving Manager to join our ambitious Fundraising team on a maternity cover basis.

In this role, you’ll lead and evolve established individual giving programmes across both charities, growing income while delivering exceptional supporter experiences.

What you’ll be doing
You’ll lead the strategic and operational delivery of individual giving across acquisition, retention and supporter care, using emotionally resonant campaigns and data driven decision making to grow our supporter base and maximise lifetime value.
Key responsibilities include
- Leading the individual giving programmes for both charities in line with our five year growth strategy
- Managing and motivating a team of three Individual Giving Officers, creating a supportive, high performing culture
- Developing engaging, personalised supporter journeys across online and offline channels
- Overseeing budgets, forecasting and performance reporting
- Managing agencies and suppliers to ensure excellent value and results
- Using insight, testing and learning to continually optimise campaigns

About you
You’ll be an experienced individual giving fundraiser who can hit the ground running and bring confidence, warmth and clarity to an interim leadership role.
You’ll have
- Strong experience delivering individual giving programmes (acquisition and retention)
- A solid track record in direct and digital fundraising, including paid social (e.g. Meta)
- A love of data and the ability to turn insight into action using CRM systems
- Experience managing budgets and external suppliers
- Previous line management experience, with a focus on wellbeing and development
- Excellent planning, project management and stakeholder engagement skills
- A test and learn mindset and a commitment to continuous improvement
Experience with MS Dynamics CRM, CRM migration, matched funding campaigns or fundraising qualifications is welcomed but not essential.

Your work will make a real difference by helping people in prison take positive steps toward recovery and a better future. You’ll play a vital role in supporting mental health, reducing risk, and empowering individuals to manage their wellbeing.
In this role, you will
- Assess and manage referrals – Accept referrals via agreed protocols, determine suitability, and ensure clients receive the right level of care, including stepping up treatment when necessary.

- Accredited PWP with BACP or BPS.
- Post-qualification experience delivering the full range of evidence-based IAPT low-intensity interventions at Step 2.
- Proven experience working within a stepped care service, supporting individuals with mental health problems and achieving recovery outcomes.
- Strong knowledge of assessment procedures, screening, referral processes, record-keeping, and evidence-based brief psychological interventions.
- Understanding and application of evidence-based psychological therapies relevant to this role.
- Ability to build effective therapeutic relationships and practice inclusively, respecting cultural diversity and individual beliefs.
- Commitment to an inclusive values base that promotes recovery and respects diversity.
- Experience using clinical outcome measures to inform and improve practice.

In this role, you’ll work alongside other Lifestyle Support Workers, therapists, and multi disciplinary teams to engage people in meaningful activities that promote wellbeing, confidence, and independence. This could include:
- Supporting individuals one to one to build confidence accessing community spaces such as gyms, groups, or public venues
- Facilitating group activities such as healthy eating, wellbeing, or lifestyle sessions including activities in the community.

Wilton Road is a warm, supportive home where adults living with mental illness are helped to live life to the fullest. With 24 hour support and a dedicated team, we work closely with each resident to build confidence, develop daily living skills, and achieve personal goals.
Every resident has a key worker who supports them on their journey towards greater independence, with the aim of eventually moving to lower level support or their own accommodation.
What you will do and achieve as a Support Worker
I empower people affected by mental illness to rebuild confidence, strengthen independence, and reconnect with their communities. Through focused, recovery based support, I help individuals set meaningful goals, overcome barriers, and access the resources they need to move forward and create lasting positive change.
Direct Support & Case Management
- Provide 1:1 key worker support tailored to individual needs.

We have exciting opportunity at our Coventry & Warwickshire IPS Employment service, where you will join our friendly and inspiring team as a IPS Employment Specialist. Your role will be a permanent position working,
37.5 hours per week field-based role.
Role will be based in the community of South Warwickshire
A good working local knowledge of all areas of South Warwickshire would of be ideal.
You will be integrated into the NHS to provide employment support for people experiencing severe mental illness with the aim of gaining employment. You will attend 6 meetings per week with local employers to build relationships and engage them regarding our service, in the above locations.
In addition, you will meet your clients each week to provide them with face-to-face support. Key to this role will also be to undertake administration duties such as producing letters, spreadsheets, and keeping various systems up to date. As part of the role, you will receive System 1 training from the NHS which will be made up of a series of 10 online sessions and a final assessment.
You will also be expected to operate Rethinks internal CRM systems on a daily basis, full training will be provided.

What will you be doing?
- Manage a caseload of between 20-30 service users at any one time who are currently off work / unemployed, receiving specialist mental health services and who wish to return to work, pro rota.
- Build relationships with clinical teams to generate referrals and create a collaborative working partnerships with NHS clinicians where employment support is integrated into mental health treatment and forms part of the Transformation plan.
- Build a multi-disciplinary approach to the return to work. For example, involve clinical staff (where relevant) in managing symptoms at work, liaise with DWP and other stake holder groups.
- To prepare individuals for a return to work by supporting service users to understand their skills, aspirations and goals through vocational profiling and action planning.
- Focus on rapid job search with the service user, whilst utilising local support networks to help them overcome their barriers to employment.
- Source job opportunities for service users through tailored job search and regular contact with employers, with 6 face to face contacts per week in the community.
Do you have what we are looking for?
- A basic understanding of IPS (Individual Placement Support) gained through experience or self-conducted research.
- Experience in recruitment or employment along with advice and guidance on employment needs
- Ability to engage with employers and build close, trusting, and productive relationships with people
- Good understanding of the local area and it’s employers
- Strong IT skills with the competency to use a range of complex systems and software
- Good understanding of the principles and practice of supported employment
- Experience of working in supported employment services is desirable
- Good time management, diary organisation, be adaptable/flexible as no two days are the same and to be able to work with minimal supervision
- Experience of working with people with mental health problems or a similar service user group is desirable
As you will be working within the community, we do need you to hold a full UK driving licence.
