Moultrie Road provides a 7 bedded, 24 hour staffed nursing service offering rehabilitation and support to people with severe mental illness. Individuals can be supported to move on into more independent accommodation with the option of move on into our CQC registered Supported Living Service. This service can be provided in one of our 3 bedded properties or in independent accommodation.

Sheffield Outreach Service provides support to adults living with mental health problems in their own homes and around the community of Sheffield. Support is delivered according to a person-centred and collaborative care plan, and individualised to each client. This can be short-term focused support, up to longer-term to provide ongoing support. Mental Health Recovery Workers support individuals on a 1:1 basis to wok towards recovery and promote independence, by setting goals and providing both practical and emotional support. The service is accessible to adults aged 18 and above, and has 6 beds with communal areas such as kitchen, lounge, and garden. The service is staffed 24 hours per day, 7 days a week and staff work in partnership with Sheffield Health and Social Care. Referrals to the Crisis House can be made via community mental health teams (CMHTs) and directly from Liaison Psychiatry at the hospital.
