Commissioning us - Crisis Outreach Support
Our Crisis and Recovery Houses have successfully enhanced the acute care pathway for people with a mental illness. Through integration and joint ownership with NHS teams we have been able to implement seamless referral, support and discharge planning via shared electronic information systems.

As a result of delivering crisis and recovery houses for a decade, we identified a need for follow-up support after people leave our services, to ensure ongoing recovery and prevent a relapse into crisis. We have been able to pilot a Crisis Outreach intervention with funding from the Stone Family Foundation. The intervention includes up to 12 weeks of additional support for individuals that have been discharged from a crisis house, helping to resolve the causes of the original crisis and implement plans to avoid future crises. Support is provided face-to-face, by telephone, or through a mixture of the two. We use Wellness Recovery Action Plans and workshops to enhance individuals’ coping skills and outcomes.
Our Crisis Outreach services have been able to move people successfully forward in their recovery from crisis, demonstrated by routinely collected outcome data, including the Mental Health Recovery Star. For every life area of these people there is an increase in Recovery Star score suggesting that positive outcomes achieved in the service are sustained and further gains are made after leaving it.
On exit from the service, individuals were asked to complete a satisfaction questionnaire. Satisfaction levels were high, with people surveyed agreeing or strongly agreeing that:
- They felt respected and treated with dignity (97%)
- Felt listened to by staff (86%)
- Felt they had been given the right kind of support (97%)
- Would use the service again if they had a similar need (85%)
