The ESA50 Questionnaire

Some people might receive Employment Support Allowance based on the medical information they provide alone.

But most people will have to fill in the ESA50 questionnaire  as part of their Work Capability Assessment.

This is so that the DWP are aware of what they are and are not able to do.

You may want to get some help filling in the questionnaire. Your local Citizens Advice Bureau or other welfare benefits advice agency should be able to help you face to face.
