We provide a range of housing services for Older Adults with memory problems and younger people with dementia who can live independently but need some level of support.
To provide person centred recovery focussed support to older people with Dementia and Memory problems and to younger people with Dementia in their own homes. Housing Support workers will provide the full range of housing related support tasks to this client group to enable the person to maintain their tenancy, recover an improved quality of life and regain or improve life skills. This is a community based service so knowledge of community services in the area is important.
You’ll join our Reconnect floating support service and help people using the service to maintain their tenancies. It’s an important role that involves a wide range of emotional and practical support. To succeed, you will need great communication skills and an organised approach.
